You are under no obligations to move forward with any coaching services because of having taken this questionnaire or having received your results. You must provide your email address for your results to be sent to you. The phone number you provide is only used in cases where there is an email error or issue. You may have your results explained to you over the phone during a free 30-minute consultation, if requested. Due to demand and the professional and personal hands-on nature of this process, your results will be sent to you within no more than 3 regular (24-hour) days. If you do not receive your results in 3 days, please check your spam or junk folder or email [email protected] as soon as possible.
All questions must be filled out per the instructions at the top of the questionnaire. All required personal information (other than the message section, which is optional) must be filled out for the form to be sent. There are absolutely no costs or fees associated with this process. After pressing the Submit Button once, please wait for the Thank You Page to appear so you do not submit multiple forms.

Your privacy and personal information are of the utmost importance. All steps and precautions have been taken to make sure that no information is accessed by anyone but Strategic Life Coaching. Your personal information and your responses are never used by, shared with, nor sold to any third parties. Your information will be used only to create your personal analysis and to send you back the results. You will never be called unless there is an issue with your email address. All personal files and results are kept offline for added security. If you decide that you would like more information and a deeper understanding of your personal results, you can request a free 30-minute coaching consultation.
